In addition to the forms listed above Heart 'N' Hands Massage utilizes a modified intake form that is specific to your session of Integrative Massage Therapy. Please feel free to print out the "First Time Client Health History form" / the "Screening Questionnare" form and the "Body Map" form as these will be helpful in assessment of your physical issues. The time used to fill out these forms will not be counted as massage time. The session clock begins when the actual massage begins.
